We work with BIRTH PROFESSIONAL BILLING to help you with your insurance needs. They can run a verification of benefits to find out your options. This service is free and no obligation to start care with us.
BEFORE FILLING OUT THIS FORM, please contact us first to see if we are available for your due date/location. Our calendar fills up fast and we would like to make sure we are available for you before checking your insurance.
Certified Professional Midwives are considered out of network with insurance providers. This means that you are responsible for the cost of care before baby is born. (usually paid monthly and due by 32 weeks of care). After care has been completed (6-8 weeks postpartum), we can submit to your insurance for possible reimbursement. Checking your insurance before care starts give you an idea if you qualify for insurance reimbursement.
